CME: Psychodynamic Theory

 

It was a great conference once more with Dr. Constantine Della leading the discussions on transference, counter-transference, and new techniques quite different form the psychoanalysis of Freud from the yesteryears. The amount of studying and upgrading that needs to do done for anyone in the mental health profession is daunting. And rightfully so because we touch lives in the most poignant ways. my key takeaways from this conference:

  1.  There are clinical studies that evidence the efficacy of psychotherapy as equal to psychotropic medications in the management of mental health cases as both impact the brain and the mind. A combination of both is ideal for DSM5 diagnosed patients .
  2. Psychodynamic therapy is no longer only about generating insight. It’s focus is also about the therapeutic relationship of the client and the therapist
  3. Two distinct techniques to use in psychodynamic approach depending on ego strength of client : uncovering versus supportive psychotherapy

May Be Broken But Never Beaten

The Philippines has been in constant turmoil these past few months. Its leader, Rodrigo Duterte and his administration, in the interest of keeping the Philippines safe, has rationalised wanton killing of drug addicts. There are thousands of reported deaths of suspected drug addicts who have not been proven guilty yet have been gunned down by those whose sworn oath is to protect lives. There are news reports from relatives of the slain that the suspects were not drug users —-some were reported as users as vendetta killing; some happened to be in the company of users; some went on some innocuous pandesal buying spree; some never took drugs, they just looked like the part. Of others who were drug users, families lament their deaths because they now have no breadwinners or because they did the drugs in order to work their 15 hour construction shift to put food on the table. For these families, the drug addicts were heroes.

Today, the Duterte administration looks into lowering the age of children who can be charged with crimes and then imprisoned —from 15 years old to 9 years old. And, as the death culture has been conscience shaping, a number of people have voted to lower the age of imprisonment.

A good size of my practice has been with adolescents who are striving to change their lives coming from backgrounds of addiction in their families, destitution, lack of education, depression because of broken families or incest, lives that are cut misshapen by schizophrenia and personality disorders. Before they sought help, they were insolent, breaking the law, causing strife within their families, creating havoc wherever they went. Yet, these same broken souls, when they received a steady dose of love, continued opportunities to improve their lives , programs of counselling & support —-in short, when everyone REFUSED TO GIVE UP ON THEM —-they successfully turned the corner. The process is arduously slow, the effort is monumental, the personal commitment is life long…but it is possible. It has been done by so many and has evidence-based programs such as the Alcoholics Anonymous Rehabilitation Programs to show proof. In my practice, for as long as the client is standing up and swinging that bat, I meet their effort with everything I have got. I owe them that because somehow, somewhere, someone did that for same thing for me, worthy of that kindness or not.

Why shouldn’t any of us take the easy way out and just ‘kill al those drug addicts & children now ? ” Because all of us, though our actions, thoughts and deeds, brought them to that situation. And we now we have a duty to get them out. Years of a war-torn life will have taken its toll, but the human being has shown a resilience to come from behind when all hope seems lost. May be broken but never beaten we are.

Every time I sit with a new client, I mentally calculate the amount of work that needs doing to heal their spirit, make whole again. Basing the program on scientific journals of treatment plans and outcomes, I feel overwhelmed : 6 to 12 months of twice a week meetings to deal with anxiety or depression; bi-weekly meetings to change behaviour or unhealthy thought patterns; one to two years to work on bulimia;  Non-ending support and after care for PTSD and Bullied kids; minimum 1 year work plan for leadership development. These past few days showed those scientific journals wrong! For reasons definitely beyond my understanding and capacity as a helper, my most challenging clients have made great strides, growing leaps and bounds. The supervisor is now a manager; the daughter who was raped finally has placed her perpetuator in jail; the young man who lived in hate and anger has moved to forgiveness; the middle-aged woman who hid her rape in shame has let go of the secret. Is the secret sauce my brilliance? my fantastic program? maybe the assessment tools? From where I am sitting, it may  part explain. But from a scientific perspective, something or Someone has to account for that outlier result. It is quite possible that the miracles of the season I see means I was never ever ever working alone. And that gives me comfort.

This all brings me to reflect on Mathew 11: Is it you Lord who is to come or are we to wait on someone else? John asks as he languishes in prison for doing God’s work. And Jesus responds, “Go and report to John what you hear and see; the blind receive sight and the lame walk, the lepers are cleansed and the deaf hear, the dead are raised up, and the poor have the Gospel preached to them. And blessed is he who does not take offense at me” (Mt. 11:4-5). He could have scolded them for doubting Him, but He does not. Instead, Jesus instructs the disciples of John to look around them, to see the suffering of so many being healed. The point behind this point seems to be that no one but the Messiah would be able to do those things.
